A good cleaning booking form captures enough detail to price and schedule the job without becoming a consultation. Start with address, home size, service type, frequency, bedrooms, bathrooms, pets, access notes, preferred time windows, and add-ons.

Keep optional questions behind conditional logic where possible. Deep-cleaning, move-out, post-construction, and recurring visits need different details.

Test the form from the customer's phone. If the customer cannot understand the price, time window, cancellation policy, and next step, the form will create more admin instead of reducing it.
